It is definitely Tadalafil I take. As noted it stops my leg cramps and seemingly similar muscle pains from Fibro.
The TENS machine can work directly to intercept pain at a specific location. However it also can trigger endorphins which are the natural pain killer. The idea of placing the electrodes in non painful locations is to trigger the endorphins more or less body wide for an overall effect. It is also postulated that dopamine can also be triggered, even in the brain which is usually a hard place to get to.
 
it is true that use of the tens machine may release endorphins however pain also releases them in response to the pain but there other are other ways to release endorphins -
  1. Exercise.
  2. Laugh.
  3. Listen to music. .
  4. Get acupuncture.
  5. Eat dark chocolate. .
  6. Have sex. .
  7. Dance.
  8. Meditate.

I cannot find any reference for the use of Tadalafil in pain management
it is used to treat pulmonary hypertension , benign prostatic hyperplasia and its better know use to treat erectile dysfunction. In fact a common side effect is back pain.
