I never have specialized in the treatment of fibromyalgia.  I do not now have an active community practice.  You have listed my home phone number as a contact number.

For at least these reasons, I ask that you remove my name from your roster of treating physicians.

Because many rheumatologists do not provide continuing treatment for patients with fibromyalgia, you should not list their names without their permisssion.
